INSTRUCTIONS

Once your registration form is validated and submitted, you will receive an automated confirmation e-mail containing:

(1) an "Application ID Number"

(2) a link to the completed registration form.  

You will need the Application ID Number and link to submit the Application Form and Supporting Documents.

DO NOT submit the registration form more than once.
Only completed registrations will be given consideration.
Applications must be received no later than 5:00 pm EST on Friday, April15, 2011.  

All information and documentation submitted by you and your referee is stored in a secure, confidential database at Perimeter Institute that can be accessed only by Equinox Summit's Forum Screening Committee.  We appreciate your cooperation with this application procedure.
